Patch Sprint Participant Agreement and Liability Waiver

All participants are required to read, sign, and date a Participant Agreement and Liability Waiver prior to participating in the Patch Sprint.  A parent or guardian must sign for all minors under the age of 18.  Please print out and bring a completed waiver form to packet pick-up on Friday night, or Saturday morning before the start of the event. * You may also Email completed form (no later than May 20)
